Forms for Approved Program Hosts
If we have approved your request to host a Speakers Bureau program, please review these materials so that you are familiar with the host’s responsibilities and are well-prepared for the event:
Before the Program
- Speakers Bureau Host Packet (PDF) (This will be emailed to you after your sign your Speakers Bureau Agreement via Docusign.)
After the Program
- Speakers Bureau Host Survey. This will be emailed to you from Submittable shortly after your program; please complete this form within two weeks of the program date. Please note that you need to finish this survey before you can book additional Speakers Bureau programs.
- Speakers Bureau Audience Survey. Please share this form with attendees and encourage them to share their feedback. (A printable version is in the Host Packet above.)
